MIGS stands for minimally invasive glaucoma surgery. MIGS procedures are the most advanced and modern forms of glaucoma lasers and surgery. The benefits of these procedures over traditional treatments of glaucoma like eyedrops and invasive surgery are numerous.

One of the primary reasons for MIGS is to prevent irreversible blindness, which is prevalent in most glaucoma cases. MIGS aims to lower high intraocular pressure, which is the cause of glaucoma. Glaucoma happens when fluid collects in the front of the eye and permanently damages the optic nerve.

How is MIGS different from traditional glaucoma surgery?

MIGS is more favourable than traditional glaucoma surgery as it is much less traumatic and much faster to perform. In some cases, MIGS provides an opportunity to remove cataracts at the same time.

How do you perform MIGS?

Primarily MIGS lowers high eye pressure and prevents damage to the optic nerve. MIGS is a collective of procedures that entails the use of microsurgical lasers, equipment, implants and tiny incisions.

Types of MIGS surgeries are:
  • Trabecular bypass MIGS: Various techniques or micro-implants are used to enhance the natural drainage of aqueous fluid from the eye.
  • Subconjunctival-MIGS: Microsurgical procedures or implants create the same drainage pathways as traditional surgery but using much smaller incisions and less invasive techniques.
  • Suprachoroidal MIGS: Micro-implants can be used to increase the outflow of aqueous fluid through non-traditional drainage pathways between the middle layers of the eye.
  • Laser MIGS: With the latest laser technology advancements, treating glaucoma can be done with ease.  Lasers can be used to either enhance the drainage of fluid from the eye or reduce the production of fluid by the eye.

Dr McClunan uses a wide range of MIGS and tailors his approach to each individual patient. He commonly performs iStent, Trabeculoplasty, High-frequency deep sclerostomy, Xen, Paul, Selective laser trabeculoplasty and Micropulse procedures.
